RESUMEN

Objective To study the mechanism of Shexiang Baoxin pill in the treatment of atherosclerosis, and to provide a theoretical basis for long-term clinical application. Methods The chemical components and targets of Shexiang Baoxin pill were collected and screened by TCMSP, TCMID, ETCM and BATMAN databases. The targets related to atherosclerosis were collected and screened by DisGeNet, OMIM, TCMSP, DrugBank and DisGeNet. Drug-compound target network and protein-protein interaction network were constructed. Go and KEGG enrichment analysis of Shexiang Baoxin pill in the treatment of atherosclerosis were carried out on MetaScape platform. Results 114 potential therapeutic components of Shexiang Baoxin pill on atherosclerosis were selected, corresponding to 175 targets. The results of network analysis showed that the main active components of Shexiang Baoxin pill were chenodeoxycholic acid, ursodeoxycholic acid, cinnamaldehyde and ginsenoside Rb1. The results of pathway enrichment showed that the anti-atherosclerotic mechanism of Shexiang Baoxin pill was related to the regulation of immunity, inflammation, and metabolism. Conclusion The active components of Shexiang Baoxin pill could act on ALB, INS, AKT1, ACTB, TNF, IL-6 and other targets, regulating multiple pathways to achieve the therapeutic effect on atherosclerosis.

ObjectiveTo investigate the therapeutic effect of the water extract of Zanthoxylum bungeanum aqueous extract(ZBAE)on rheumatoid arthritis. MethodThe sixty SD rats were divided into normal group, model group [complete Freund's adjuvant (CFA), 10 mg·kg-1], methotrexate(MTX) group (0.25 mg·kg-1), low -, medium -, and high-dose ZBAE groups (90, 180, 360 mg·kg-1). The rats in MTX group were given intraperitoneal injection for two weeks, three times a week, and the rats in ZBAE group were administrated for 14 days. The swelling of the ankle joint and body weight were observed, and arthritis scores were also performed. Computed tomography (CT), hematoxylin-eosin (HE) staining and safranine-O and fast green staining were used to observe the effect of ZBAE on synovial hyperplasia and bone protection. Cell counting kit-8(CCK-8)method was used to detect the proliferation of the RA-FLSs cells treated with ZBAE. According to the results of CCK-8 experiment, the optimal concentration and time of administration were determined, blank group, low -, medium -, and high-dose ZBAE groups (0.08,0.10,0.12 g·L-1) were set up. The cell cycle distribution and apoptosis rate were analyzed by flow cytometry,the migration ability of RA-FLSs cells was examined by scratch test. Western blot was used to detect the effect of ZBAE on phosphatidylinositol 3-kinase (PI3K), protein kinase B (Akt), cyclin-dependent kinase 2 (CDK2), Cyclin A and phosphorylated PI3K, Akt (p-PI3K,p-Akt) protein expression in RA-FLSs cells. ResultCompared with the normal group,joint swelling index and arthritis score were increased in the model group (P<0.05),the bone of the ankle was seriously damaged, and there was obvious synovial hyperplasia. Compared with the model group, the ZBAE group could significantly reduce the joint swelling index (P<0.05), inhibit synovial hyperplasia and cartilage destruction. In vitro study showed that compared with the blank group, ZBAE could inhibit the migration of RA-FLSs (P<0.05), promoted cell apoptosis (P<0.05), and acted on RA-FLSs cells in S phase to inhibit cell proliferation. Moreover, the result of Western blot showed that compared with the blank group, the expression of p-PI3K, p-Akt, CDK2 and Cyclin A proteins were significantly decreased in the high dose group of ZBAE (P<0.05). ConclusionThese results suggest that ZBAE has a therapeutic effect on rheumatoid arthritis by inhibiting synovial hyperplasia, promoting synovial apoptosis and inhibiting its migration.

GI (GIGANTEA) is one of the output key genes for circadian clock in the plant. The JrGI gene was cloned and its expression in different tissues was analyzed to facilitate the functional research of JrGI. RT-PCR (reverse transcription-polymerase chain reaction) was used to clone JrGI gene in present study. This gene was then analyzed by bioinformatics, subcellular localization and gene expression. The coding sequence (CDS) full length of JrGI gene was 3 516 bp, encoding 1 171 amino acids with a molecular mass of 128.60 kDa and a theoretical isoelectric point of 6.13. It was a hydrophilic protein. Phylogenetic analysis showed that JrGI of 'Xinxin 2' was highly homologous to GI of Populus euphratica. The result of subcellular localization showed that JrGI protein was located in nucleus. The JrGI, JrCO and JrFT genes in female flower buds undifferentiated and early differentiated of 'Xinxin 2' were analyzed by RT-qPCR (real-time quantitative PCR). The results showed that the expression of JrGI, JrCO and JrFT genes were the highest on morphological differentiation, implying the temporal and special regulation of JrGI in the differential process of female flower buds of'Xinxin 2'. In addition, RT-qPCR analysis showed that JrGI gene was expressed in all tissues examined, whereas the expression level in leaves was the highest. It is suggested that JrGI gene plays a key role in the development of walnut leaves.

Objective: To investigate the clinical characteristics of abnormal liver function in patients with advanced esophageal squamous carcinoma treated with programmed death-1 (PD-1) antibody SHR-1210 alone or in combination with apatinib and chemotherapy. Methods: Clinical data of 73 patients with esophageal squamous carcinoma from 2 prospective clinical studies conducted at the Cancer Hospital Chinese Academy of Medical Sciences from May 11, 2016, to November 19, 2019, were analyzed, and logistic regression analysis was used for the analysis of influencing factors. Results: Of the 73 patients, 35 had abnormal liver function. 13 of the 43 patients treated with PD-1 antibody monotherapy (PD-1 monotherapy group) had abnormal liver function, and the median time to first abnormal liver function was 55 days. Of the 30 patients treated with PD-1 antibody in combination with apatinib and chemotherapy (PD-1 combination group), 22 had abnormal liver function, and the median time to first abnormal liver function was 41 days. Of the 35 patients with abnormal liver function, 2 had clinical symptoms, including malaise and loss of appetite, and 1 had jaundice. 28 of the 35 patients with abnormal liver function returned to normal and 7 improved to grade 1, and none of the patients had serious life-threatening or fatal liver function abnormalities. Combination therapy was a risk factor for patients to develop abnormal liver function (P=0.007). Conclusions: Most of the liver function abnormalities that occur during treatment with PD-1 antibody SHR-1210 alone or in combination with apatinib and chemotherapy are mild, and liver function can return to normal or improve with symptomatic treatment. For patients who receive PD-1 antibody in combination with targeted therapy and chemotherapy and have a history of long-term previous smoking, alcohol consumption and hepatitis B virus infection, liver function should be monitored and actively managed in a timely manner.

Diabetes is a metabolic disease mainly characterized by hyperglycemia due to inadequate insulin secretion. And persistent hyperglycemia can cause chronic damage or dysfunction of eyes, kidneys, heart, blood vessels and nerves. Polysaccharides are high molecular carbohydrates polymerized by glycosidic bonds from more than 10 monosaccharide molecules of the same or different types. They have the advantages of wide sources, high safety and low toxic and so on. As one of the important effective components of traditional Chinese medicine, polysaccharides have biological activities such as immune regulation, anti-oxidation, anti-tumor, lowering blood sugar and so on. The structure is directly related to biological activities, and the advanced structure of polysaccharides is based on the primary structure. Exploring the primary structure of polysaccharides is the key task of lowering blood sugar and improving diabetic complications. This paper summarizes the monosaccharide composition of the primary structure of Chinese medicine polysaccharides, and the mechanism of Chinese medicine polysaccharides improving diabetes is emphasized by increasing the secretion and release of insulin, increasing the islet β cell number, upregulating insulin receptor level, improving glucose and lipid metabolism, inhibiting inflammatory response, improving oxidative stress and regulating phosphatidylinositol-3-kinase(PI3K)/protein kinase B (Akt), mitogen activated protein kinase, cyclic adenosine monophosphate(cAMP)/protein kinase A(PKA) and adenosine monophosphate activated protein kinase(AMPK) signaling pathways. At the same time, we also summarized the prevention and treatment of Chinese medicine polysaccharides in diabetic nephropathy, diabetic cardiomyopathy, diabetic ophthalmopathy and diabetic peripheral neuropathy, in order to provide a theoretical basis for new drug development and clinical application of Chinese medicine polysaccharides in the intervention of diabetes and its complications.

ObjectiveTo observe the clinical efficacy and safety of Chinese medicinal mixture for dispelling stasis and resolving phlegm combined with western medicine in the treatment of epilepsy combined with cognitive impairment by randomized, double-blind, placebo-controlled, parallel-group trial. MethodA total of 123 inpatients and outpatients with epilepsy complicated with cognitive impairment admitted to the department of neurology at Longhua Hospital from October 2020 to October 2021 were randomly assigned into a control group (62 cases, carbamazepine + placebo) and a treatment group (61 cases, carbamazepine + Quyu Dingxian Zhengtong mixture) by random number table method. In the treatment group, 4 cases were exfoliated and eliminated. In the control group, 3 cases fell off. Finally, 57 cases in the treatment group and 59 cases in the control group were included. The total course of treatment for both groups was 12 weeks. The clinical efficacy, efficacy for traditional Chinese medicine (TCM) syndromes, and incidence of adverse reactions were compared between two groups. The changes of seizure frequency, abnormal rate of electroencephalogram (EEG), cognitive function score, serum homocysteine (HCY), folic acid, and vitamin B12 (B12) were measured and compared before and after treatment. ResultAfter 12 weeks of treatment, the treatment group had higher clinical efficacy [92.98% (53/57) vs 79.66% (47/59), χ2=4.327, P<0.05] and efficacy for TCM syndromes [96.49% (55/57) vs 84.75% (50/59), χ2=4.660, P<0.05] than the control group. The treatment group was superior to the control group in reducing the seizure frequency (Z=-3.938, P<0.01) and improving the Montreal cognitive assessment (MoCA) score (t=4.333, P<0.01) and mini-mental state examination (MMSE) score (t=9.531, P<0.01). The variations in serum HCY, folic acid, and B12 in the treatment group were less than those in the control group (t=-7.233, t=-7.972, t=-6.871, P<0.01). After treatment, the abnormal rate of EEG in the treatment group was lower than that in the control group (χ2=4.437, P<0.05). The incidence of adverse reactions in the treatment group (1.75%, 1/57) was lower than that (13.56%, 8/59) in the control group (corrected χ2=4.116, P<0.05). ConclusionChinese medicinal mixture for dispelling stasis and resolving phlegm in combination with western medicine had better efficacy and safety than western medicine alone in the treatment of epilepsy complicated with cognitive impairment. Specifically, the combination outperformed western medicine alone in terms of clinical efficacy, efficacy for TCM syndromes, reduction in seizure frequency, abnormal rate of EEG, adverse reactions, improvement of cognitive function, and variations in serum folic acid, B12, and HCY values. Chinese medicinal mixture for dispelling stasis and resolving phlegm may improve the clinical efficacy and safety by changing the metabolism of folic acid, B12, and HCY in serum of the patients with epilepsy complicated with cognitive impairment.

The clinical features and gene mutation characteristics of 2 children who suffered from Cornelia de Lange syndrome(CdLs)and were admitted to Affiliated Hospital of Zunyi Medical University in March 2019 were retrospectively analyzed.The 2 cases developed in infancy and presented with intractable epilepsy were accompanied with developmental retardation and special appearance.It was obvious that the 2 children had SMC1A gene mutations on X chromosome.Case 1 was frameshift mutations in the SMC1A gene at c. 2561dupA(p.K854fs), and case 2 was mutations in the SMC1A gene at c. 3441+ 1G>A(exon24)splicing mutation.These were heterozygous de novo mutations, and weren′t detected in their parents, which was not reported in literatures.In this study, 2 cases of CdLs were caused by SMC1A gene mutation, which enriched the human gene mutation database.CdLs should be considered in children with early-onset epilepsy, especially appearance and developmental retardation.Genetic testing is the most important diagnostic method.

The global morbidity and mortality of cardiovascular diseases remain high. Atherosclerosis is an important pathological basis of cardiovascular diseases, and its pathogenic mechanism has not been fully clarified. It was reported that pathogenic mechanism of atherosclerosis is related to vascular endothelial cell injury, lipid metabolism disorder, inflammatory reaction, imbalance between autophagy and apoptosis, et al. Traditional Chinese medicine(TCM) formula has shown good effects in the prevention and treatment of atherosclerosis. There are a lot of studies that showed the anti-atherosclerosis effect and the mechanism of TCM formula. In this paper, we reviewed the mechanism of anti-atherosclerosis action of TCM formula by summarizing the research literatures in the past five years, and provide reference for the further systematic study of anti-atherosclerosis effect of TCM formula.

BACKGROUND@#Depression in Parkinson's disease (dPD) is closely related to quality of life. Current studies have suggested that Pingchan Granule (PCG) might be effective for treating dPD.@*OBJECTIVE@#This study determines the efficacy of PCG for depressive symptoms in Parkinson's disease (PD).@*DESIGN, SETTING, PARTICIPANTS AND INTERVENTIONS@#This was a randomized, double-blind, placebo-controlled trial, conducted in Longhua Hospital, Shanghai, China. Patients diagnosed with idiopathic PD and clinically significant depressive symptoms (defined by a 24-item Hamilton Rating Scale for Depression [HAM-D] score ≥ 8) were included in this study, randomly assigned to PCG or placebo group in a 1:1 ratio and followed for 24 weeks.@*MAIN OUTCOME MEASURES@#The primary outcome was the change from baseline to week 24 in HAM-D score among the set of patients who completed the study following the treatment protocol (per-protocol set). Secondary outcomes included changes in scores on the Unified Parkinson's Disease Rating Scale (UPDRS) part 2 (UPDRS-II), UPDRS part 3 (UPDRS-III), Parkinson's Disease Sleep Scale (PDSS) and Hamilton Rating Scale for Anxiety (HAM-A), between baseline and week 24.@*RESULTS@#Eighty-six patients were enrolled, and 85 patients were included in the per-protocol set. HAM-D scores decreased by an adjusted mean of 11.77 (standard error [SE] 0.25) in the PCG group and 3.86 (SE 0.25) in the placebo group (between-group difference = 7.91, 95% confidence interval [7.22, 8.80], P < 0.001), in the multivariable linear regression. Improvements in scores on the UPDRS-II, UPDRS-III, PDSS, and HAM-A scales were also observed.@*CONCLUSION@#Treatment with PCG was well tolerated and improved depressive symptoms and motor and other non-motor symptoms in PD.@*TRIAL REGISTRATION@#Chinese Clinical Trial Register: ChiCTR-INR-17011949.

Objective@#Hyperbaric oxygen treatment (HBOT) has demonstrated efficacy in improving hearing levels of patients with idiopathic sudden sensorineural hearing loss (ISSHL); however, the underlying mechanisms are not well understood. HBOT alleviates the inflammatory response, which is mediated by Toll-like receptor (TLR) 4 and nuclear factor (NF)-κB. In this study we investigated whether HBOT attenuates inflammation in ISHHL patients alteration of TLR4 and NF-κB expression.@*Methods@#ISHHL patients ( = 120) and healthy control subjects ( = 20) were enrolled in this study. Patients were randomly divided into medicine group treated with medicine only ( = 60) and HBO group receiving both HBOT and medicine ( = 60). Audiometric testing was performed pre- and post-treatment. TLR4, NF-кB, and TNF-α expression in peripheral blood of ISSHL patients and healthy control subjects was assessed by ELISA before and after treatment.@*Results@#TLR4, NF-κB, and TNF-α levels were upregulated in ISSHL patients relative to healthy control subjects; the levels were decreased following treatment and were lower in the HBO group than that in the medicine group post-treatment ( < 0.05 and < 0.01).@*Conclusion@#HBOT alleviates hearing loss in ISSHL patients by suppressing the inflammatory response induced by TLR4 and NF-κB signaling.

This study aimed to evaluate the clinical performance of p16/Ki-67 dual staining for triage high risk HPV (HR-HPV) infected women. Target objects were women who infected HR-HPV and received colposcopy examination between April and December of 2016 at the Second Affiliated Hospital of Zhengzhou University. Gynecologists collected the cervical exfoliated cells from eligible women for p16/Ki-67 dual staining, LBC testing and HPV DNA testing. Histology diagnosis were used as gold standard. Sensitivities, specificities, positive predictive values (PPVs), negative predictive values (NPVs) of p16/Ki-67 dual staining, LBC testing and HPV16/18 testing for triage of HR-HPV positive population were calculated and compared. A total of 295 HR-HPV infected women were selected, and the mean age was (44.29±11.48) years old. Positive rates of p16/Ki-67 dual staining, HPV16/18 testing and LBC testing were 70.17% (207), 56.95% (168) and 85.76% (253), respectively. When CIN2+as the endpoint, among the three triage methods, sensitivity of p16/Ki-67 dual staining was 90.00% (95: 85.06%-93.43%), higher than the value of HPV 16/18 testing, but lower than the value of LBC testing. Specificity, PPV and NPV of p16/Ki-67 dual staining were the highest [71.58% (95: 61.81%-79.67%), 86.96% (95:81.69%-90.88%) and 77.27% (95: 67.49%-84.78%)]. When detection for CIN3+, sensitivity of p16/Ki-67 dual staining was 92.90% (95: 87.74%-95.99%), lower than the value of LBC testing, but higher than the value of HPV16/18 testing. Specificity of p16/Ki-67 dual staining was 55.00% (95: 46.74%-63.00%), lower than the value of HPV16/18 testing, but higher than the value of LBC testing. PPV of p16/Ki-67 dual staining was 69.57% (95: 62.99%-75.43%), lower than the value of HPV 16/18 testing, but higher than the value of LBC testing. NPV of p16/Ki-67 dual staining was 87.50% (95: 78.99%-92.87%), higher than value of HPV 16/18 testing, but lower than the value of LBC testing. p16/Ki-67 dual staining has better clinical effects than HPV 16/18 testing and LBC testing for triage women with HR-HPV infection.

Objective To investigate the clinical value of a new-generation cartridge Xpert MTB/RIF Ultra assay on detection of Mycobacterium tuberculosis(MTB)and rifampin(RIF)resistance.Methods A total of 111 patients from He'nan Provincial Chest Hospital with suspected tuberculosis(TB)and retreated TB were enrolled into this study from March to December 2016,including 33 cases of tuberculosis detection group(CDG)and 78 cases of drug resistant high-risk group(DRG).The sputum samples of patients were collected.The sensitivity and specificity of Xpert MTB/RIF Ultra,sputum smear,solid Lowenstein-Jensen(L-J)culture and mycobacterial growth indicator tube(MGIT)culture for MTB were evaluated.RIF resistance was performed by Xpert MTB/RIF Ultra,traditional phenotypic drug sensitivity test and Xpert MTB/RIF assay.Measurement data were compared using t-test,and categorical data were compared using chi-squared test.Results Using clinical diagnosis result as the standard,in the CDG,the sensitivity of Xpert MTB/RIF Ultra for MTB detection(75.8%)was not significantly different from those of sputum smear(66.7%),L-J culture(63.6%),MGIT culture(75.6%)and Xpert MTB/RIF(66.7%)(x2=0.67,1.15,0.00 and 0.67,respectively,all P>0.05).In the DRG,the sensitivity of Xpert MTB/RIF Ultra(94.9%)was better than those of L-J culture(55.1%)and MGIT culture(80.8%)with statistical significance(x2=32.8 and7.25,respectively,both P <0.05).The sensitivity of Xpert MTB/RIF Ultra was not significantly different from sputum smear(84.6%)and Xpert MTB/RIF(91.0%)(x2=3.41 and 0.39,respectively,both P>0.05).Xpert MTB/RIF Ultra took the shortest time to obtain the final results,which was(1.76±0.18)h and significantly shorter than smear test([5.04±0.49]h),L-J culture([31.67±0.56]h),MGIT culture([22.36±9.68]h),Xpert MTB/RIF([2.00±0.30]h)(t=16.90,31.98,24.38 and 7.05,respectively,all P <0.01).Using culture result as the standard,the sensitivity for MTB detection of Xpert MTB/RIF and Xpert MTB/RIF Ultra were 93.2% and 98.9%,and the specificity of Xpert MTB/RIF and Xpert MTB/RIF Ultra were both 100%.The sensitivity for MTB detection of Xpert MTB/RIF Ultra(52.2%)was significantly better than that of Xpert MTB/RIF(21.7%)in 23 smear-negative pulmonary TB patients with statistical significance(x2=4.98,P=0.025).Using traditional drug susceptibility test as the standard,the sensitivities for RIF resistance detection of Xpert MTB/RIF and Xpert MTB/RIF Ultra in culture-positive TB patients were 90.9% and 93.2%,respectively,and the specificities were 89.5% and 92.9%,respectively.Conclusions Xpert MTB/RIF Ultra has a higher MTB detection rate than Xpert MTB/RIF in smear-negative pulmonary TB patients.In drug-resistant pulmonary TB patient,MTB/RIF Ultra has high sensitivity,and it takes shorter time to detect MTB and RIF resistance.Thus,Xpert MTB/RIF Ultra has a good application prospect in clinical work.

Objective To explore the independent risk factors of the prolonged time of retention ICU after type A aortic dissection operation,to establish a corresponding perioperative risk assessment system.Methods A total of 509 consecutive patients with type A aortic dissection were enrolled in the study from September 2011 to May 2016,among which,418 patients received emergency operation.The prolonged retention time of ICU was considered as endpoint event.A perioperative assessment system was established through the analysis of related risk factors of the most common complications after type A aortic dissection operation.The possible risk factors of prolonged retention time of ICU were introduced into a single factor analysis.The meaningful variables in the single factor analysis were introduced into logistic regression analysis.The independent risk factors which were obtained from logistic regression analysis were used to establish risk prediction modeL,and the ROC curve and Hosmer-Lemeshow goodness of fit test were used to evaluate the model.Results The perioperative mortality rate was 8.64%,the prolonged retention time of ICU was 5.06 days,and 98 cases exceeded 7 days.The results of logistic regression analysis showed that age,the history of stroke,obesity,emergency operation,cardiopulmonary bypass time,deep hypothermic circulatory arrest time,renal inadequacy,massive transfusion,hypoxemia,and pulmonary infection were the independent risk factors for prolonged retention time of ICU.Accordingly,a mathematical model was established.The area under ROC cure for prediction model(AUC) =0.761,Hosmer-Lemeshow goodness of fit test P =0.512.Conclusion The logistic model estabhshed in this study can successfully predict ICU retention time after type A aortic dissection operation,and the efficacy was predicted satisfactorily.

<p><b>Objective</b>Human papilloma virus (HPV) is a necessary cause of cervical cancer and is also closely related to penile cancer, oropharyngeal cancer, and anal cancer in males. However, few studies are reported on male HPV. This study aimed to investigate HPV infection of the external genitalia in men whose female partners have cervical HPV infection.</p><p><b>METHODS</b>We collected the relevant data on the male outpatients whose partners had cervical HPV infection in our Department of Urology and Andrology from August to December 2016. We obtained samples with nylon swabs from the glans penis, corona, inner layer of the prepuce and penile body and detected different types of HPV infection using the Hybribio HPV typing kit, PCR and membrane hybridization.</p><p><b>RESULTS</b>Valid data were collected from 140 males, which showed 83.5% of HPV infection of the external genitalia, including 60 cases of HPV6 (43.2%), 27 cases of HPV16 (19.4%), 14 cases of HPV39 (10.1%), 13 cases of HPV18 (9.4%), 13 cases of HPV58 (9.4%), and 13 cases of HPV52 (9.4%). Redundant prepuce was found in 75.5% of the males, but there was no statistically significant difference in the incidence rate of HPV infection between the normal and redundant prepuce groups (P > 0.05).</p><p><b>CONCLUSIONS</b>Men who have the female partners with positive cervical HPV are at high risk of HPV infection and therefore need to be screened and treated so as to reduce HPV infection in both sexes.</p>

Emerging evidence has indicated that circular RNAs (circRNAs) play pivotal roles in the regulation of cellular processes and are found to be aberrantly expressed in a variety of tumors.However,the clinical role of circRNAs in bladder cancer (BC) and the molecular mechanisms have yet to be fully understood.In this study,the clinical specimens were obtained and the expression level of a circRNA BCRC4 was detected by real-time PCR in both BC tissues and cell line.The circular RNA over-expression plasmid was constructed and transfected into BC cells and related cell line.The cell cycles and apoptosis were observed using inverted microscope and flow cytometry.Western blotting was used to compare the relative protein expression of groups with different treatments.It was found that circRNA BCRC4 expression was lower in BC tissues than in adjacent normal tissues.Furthermore,consequences of fomed-expression of BCRC4 promoted apoptosis and inhibited viability of T24T and UMUC3 cells,and up-regulated BCRC4-inereased miR-101 level,which suppressed EZH2 expression in both RNA and protein levels.In addition,gambogic acid (GA) is a promising natural anticancer compound for BC therapy,and GA treatment increased the BCRC4 expression in T24T and UMUC3 cells in a dose-dependent manner.Altogether,our findings suggest that BCRC4 functions as a tumor suppressor in BC,and mediates anticancer function,at least in part,by up-regulating the expression of miR-101.Targeting this newly identified circRNA may help us develop a novel strategy for treating human BC.

Chronic obstructive pulmonary disease (COPD) is a chronic respiratory disease characterized by incompletely reversible airflow limitation and progressive course.The pathogenesis of COPD is complicated.Clinical symptoms include declining pulmonary function, airway and lung inflammation, cough, emaciation, low activity and so on.In the past, the medications for COPD treatment were mainly bronchodilators and anti-inflammatory drugs.With the better understanding of its pathogenesis, anti-oxidative drugs have also been used in the clinical treatment of COPD recently.This article reviews the development of drugs which include bronchodilators, anti-inflammatory drugs and anti-oxidative drugs for the therapy of COPD.

Objective To observe the clinical efficacy of moxibustion at He-Sea plus Front-Mu points in treating chronic atrophic gastritis (CAG) due to deficient cold in spleen-stomach, and its effect on serum pepsinogen (PG) and gastrin. Method Sixty-three eligible patients with CAG due to deficient cold in spleen-stomach were divided into a control group (31 cases) and a treatment group (32 cases) by using random number table. The control group was intervened by Wei Fu Chun tablets, while the treatment group received moxibustion at He-Sea plus Front-Mu points, 12 weeks as a treatment course. Clinical efficacy, symptoms score of traditional Chinese medicine (TCM), serum PGⅠ, PG Ⅱ and gastrin levels were evaluated. Result The total effective rate was 93.8% in the treatment group, significantly better than that in the control group (P<0.05); the TCM symptoms scores dropped significantly in both groups after the treatment and in the follow-up study (P<0.01), and the treatment group was markedly superior to the control group (P<0.01); the levels of PG Ⅰ, PG Ⅰ/Ⅱ ratio (PGR) and gastrin-17 (G-17) increased significantly in the treatment group after the intervention (P<0.01), and PG Ⅱ dropped significantly (P<0.05); after the treatment, thelevels of PG I, PGR and G-17 increased significantly in the control group (P<0.05); after the intervention, the treatment group was significantly better than the control group in comparing the levels of PG Ⅰ, PGR and G-17 (P<0.01), and there was a significant difference in comparing the level of PG Ⅱ between the two groups after the intervention (P<0.05). The follow-up showed that the HP positive rate was 3.1% in the treatment group, significantly lower than that in the control group (P<0.05). Conclusion Moxibustion at He-Sea plus Front-Mu points can significantly improve CAG symptoms, enhance HT clearance and lower the relapse; its action mechanism is possibly through up-regulating PGⅠ, PGR and G-17 and down-regulating PG Ⅱ.

Objective: To observe the clinical effects of moxibustion at abdominal acupoints for slow transit constipation (STC) due to yang deficiency of the spleen and kidney. Methods: A total of 52 cases with slow transit constipation in conformity with the inclusion criteria were selected and divided into a control group and an observation group according to their visit order and random digital table, 26 cases in each group. Patients in the control group received routine nursing guide. Besides the same routine nursing guide, patients in the observation group received moxibustion at the abdominal acupoints, once every day. The course of the treatment was 4 weeks in the two groups, and the 3-month follow-up was given after the course was finished, for comparing the clinical symptoms, results of colon transit tests, scores of depression/anxiety scale and nursing satisfaction. Results: The total effective rate was 92.3% in the observation group and 69.2% in the control group, with a significant difference between the two groups. After the treatment and during the follow-up checks, the scores of Chinese medical symptoms in the two groups were remarkably decreased than those before the treatment (all P<0.01); the scores of the observation group were obviously lower than those in the control group (all P<0.01). The discharge rates of the markers in the two groups were remarkably increased than those at the same time period before the treatment; moreover, the discharge rates of the markers at various time periods were remarkably better in the observation group than those in the control group (P<0.01). SDS and SAS scores were remarkably decreased after the treatment in the two groups (P<0.01). In comparison of SDS score between the two groups after the treatment, the difference was statistically significant (P<0.01). In comparison of SAS score between the two groups after the treatment, the difference was statistically significant (P<0.05). The nursing satisfaction was 96.2% in the observation group after the treatment, obviously better than that in the control group (73.1%). The recurrence rate was 8.3% in the observation group, remarkably lower than that in the control group (33.3%). Conclusion: Moxibustion at the abdominal acupoints plus routine nursing can remarkably improve the colon transit functions and anxious and depressive emotion in patients with STC, and the therapeutic effects are remarkable. Not only the clinical satisfaction is higher, but the recurrence rate is obviously lower than that of routine nursing.

<p><b>Objective</b>To analyze the clinical characteristics of secondary male hypogonadism induced by sellar space-occupying lesion, explore its pathogenesis, and improve its diagnosis and treatment.</p><p><b>METHODS</b>We retrospectively analyzed the clinical data about 22 cases of secondary male hypogonadism induced by sellar space-occupying lesion, reviewed related literature, and investigated the clinical manifestation, etiological factors, and treatment methods of the disease. Hypogonadism developed in 10 of the patients before surgery and radiotherapy (group A) and in the other 12 after it (group B). The patients received endocrine therapy with Andriol (n=7) or hCG (n=15).</p><p><b>RESULTS</b>The average diameter of the sellar space-occupying lesions was significantly longer in group A than in B ([2.35±0.71] vs [1.83±0.36] cm, P<0.05) and the incidence rate of prolactinomas was markedly higher in the former than in the latter group (60% vs 0, P<0.01). The levels of lutein hormone (LH), follicle stimulating hormone (FSH) and testosterone (T) were remarkably decreased in group B after surgery and radiotherapy (P<0.01). Compared with the parameters obtained before endocrine therapy, all the patients showed significant increases after intervention with Andriol or hCG in the T level ([0.78±0.40] vs [2.71±0.70] ng/ml with Andriol; [0.93±0.44] vs [3.07±0.67] ng/ml with hCG) and IIEF-5 score (5.00±2.61 vs 14.50±3.62 with Andriol; 5.36±1.82 vs 15.07±3.27 with hCG) (all P<0.01). The testis volume increased and pubic hair began to grow in those with hypoevolutism. The patients treated with hCG showed a significantly increased testis volume (P<0.01) and sperm was detected in 7 of them, whose baseline testis volume was markedly larger than those that failed to produce sperm ([11.5±2.3] vs [7.5±2.3] ml, P<0.01). Those treated with Andriol exhibited no significant difference in the testis volume before and after intervention and produced no sperm, either.</p><p><b>CONCLUSIONS</b>Hypothyroidism might be attributed to surgery- or radiotherapy-induced damage to the pituitary tissue, space-occupying effect of sellar lesion, and hyperprolactinemia. Both Andriol and hCG can improve the T level and erectile function, but the former does not help spermatogenesis.</p>

<p><b>Objective</b>To investigate the relationship between the abnormal sperm DNA methylation level and early spontaneous abortion.</p><p><b>METHODS</b>We randomly selected 98 males who met the inclusion criteria and whose wives suffered from unexplained abortion or embryo abortion, and included another 46 normal healthy men present for pre-pregnancy check-up as controls. We examined the semen quality and sperm morphology, obtained the sperm DNA fragmentation index (DFI) by modified sperm chromatin dispersion, and measured the sperm DNA methylation level using the methylated DNA quantification kit and the colorimetric method.</p><p><b>RESULTS</b>Compared with the normal controls, the men in the unexplained abortion group showed a significantly lower rate of big-halo sperm ([45.50 ± 26.27] vs [36.49 ± 23.06]%, P = 0.038), a higher rate of abnormal-head sperm ([77.08± 12.21] vs [81.09± 10.89]%, P = 0.049), and a lower level of sperm DNA methylation ([0.47 ± 0.33] vs [0.36 ± 0.26] ng/μl, P = 0.035). The sperm DNA methylation level was positively correlated with the percentage of big-halo sperm (OR=0.546, P<0.01). Multivariate regression analysis manifested that sperm head abnormality was an independent risk factor of early spontaneous abortion or embryo abortion (OR=1.032, P = 0.049), while the high methylation level was protective factor against early spontaneous abortion or embryo abortion (OR=0.244, P = 0.03).</p><p><b>CONCLUSIONS</b>The abnormal level of sperm DNA methylation may be one of the important reasons for early spontaneous abortion or embryo abortion.</p>
